Lost Original i-194

I lost my original cover letter and i-194, I have looked on all the US websites and cannot find way to have a new one issued, any idea?

Wow I wish, I am assuming you are also Canadian with a record. This thing has cost me close to 2500.00 so far, also you say you have a I-94, that is different from a I-194

So weird. The first time I crossed I had to go in to the building in Blaine WA crossing and pay 6 bucks American for a I 94 visa card and stamp. They attached it to my passport and told me I have to return it and get a new one in 6 months.

I also just received the letter, no card to attach to passport. Received waiver this January.

Yes I am Canadian and I did not get a card for my passport and only the cover letter as well as the I-194 which must be presented when entering the USA

Weird that you don't have the 6 dollar visa card. I have a letter also and need both to cross. Unless you aren't Canadian?

Mine is a letter and it does not get stapled in your passport, you show it to customs with your passport. The waiver only applies to the USA and you do not get questioned in any other country. No they have never asked me about the charges. This waiver is required by US customs only. I lost the original and had a copy with me when going through Houston, caused some issues so I am trying to get it replaced. This is a I-194 waiver. I would also have it with you travelling to other countries in case your flight has to divert on US Soil. This has also happened to me.

It's a visa card stapled in to your passport. Under the card is a visa stamp with the expiry date (6months) you could very easily take the card out when traveling in other countries and simply staple it back before entering the u.s. I'd be shocked if other countries knew the stamp was a waiver stamp as it just says i-94 visa.

Hello Coleen,

I hope you are well. I was wondering, does the waiver actually get glued into your passport or is it a document you keep with you in your carry on? I travel frequently around the world and I am concerned other countries will deny me entry when customs and immigration officials see the waiver. Lastly, do officers still interrogate you about your charges when you clear US Customs?
